We would like to thank you for taking part in our new process of accepting athletic participation paperwork online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.

ACCESS FORMS: You must fill out EACH form

*To access the online forms click the “Electronic Participation Forms” tab. You will get a list of the forms. Must use athlete’s legal name as registered at school.

*Click on the form name and fill out the information requested. All information must be completed.

SIGN & SUBMIT FORMS:

*To sign the document click inside the signature box and hold your mouse button down. This will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box.

*Once you have filled out all of the information on each page you will have the opportunity to print the document if you wish.

*All forms are required to be filled out each school year.

PRINT PHYSICAL FORM:

*The NCHSAA Pre-participation physical is required and will continue to be collected as a hard copy. Click on ‘download & print’ to print a copy to bring to the physician. Physicals are valid for 395 days.
